After-Death Bereaved Family Interview

    The instruments in PDF format require Adobe Acrobat Reader 4.0 or greater.  If you do not have Acrobat Reader installed on your computer, click here to download it for free. The DOC format requires Microsoft Word (or other word processing application with the ability to read MS Word files).

    If you plan to employ one of the Toolkit instruments, please complete the Toolkit Registration Form and return to Dr. Joan M. Teno, MD, MS [Center for Gerontology & Health Care Research, Brown University School of Medicine, 167 Angell Street, Lower Level, Providence, RI 02912 or by fax at 401-863-1742]. Upon receipt of this registration form, Dr. Teno will send the necessary information for scoring the TIME instruments to the applicant.
